There are a few details to take care of before sending your child off to Free Kick Soccer Camp. Please read all sections carefully to ensure everything goes smoothly.

There is a total of three forms that must be filled out prior to enrollment in the camp. The t-shirt form can be submitted directly through this page. However, please send the other two completed forms (COVID-19 Safety & General Consent) to freekicksoccercamp@gmail.com. Once all forms are received you will shortly receive a confirmation email that you are all signed up!
